Publications

The Department is currently updating informational brochures to reflect the rules that that took effect in August 2023.

Any publication appearing with a form number in the list below, is a printed publication.  Publications appearing without a form number, are available in electronic format only. All printed publications may be ordered, free-of-charge from the Department’s warehouse using the DH-48 Request for Forms. Be aware that some publications have limits on the number of copies that may be ordered. Someone must be at the shipment address to receive the UPS shipment or else it will be returned to the Department’s warehouse as undeliverable. If a shipment is returned because the business was not open during normal business hours, the Department will not process any more requests for materials from that entity.
