Facility Agent ID Fingerprint Submission Information

Notice: As of December 8, 2022, the department will no longer require facility agent ID applicants to submit fingerprints for a criminal history background check.

NOTICE: Registration Number for facility agent ID applicants: 8222

All owners who hold any portion of the economic or voting interest of a medical marijuana facility who will also have access to medical marijuana or the medical marijuana facility other than as an escorted visitor, and all officers, directors, board members, managers and employees must apply for a facility agent ID card and must submit fingerprints for a criminal record check in order for that application to be considered complete.

Waiver issued pursuant to 19 CSR 30-95.025(2)(A):

  • Individuals who submitted fingerprints as part of a facility application will not be required to resubmit fingerprints as part of the facility agent ID application. Such individuals have already submitted fingerprints for purposes of licensing and need not resubmit fingerprints. Therefore, 19 CSR 30-95.040(3)(E)1.B that requires an attestation affirming submittal of fingerprints for a background check within the last six months is hereby waived insomuch as that rule would require this class of individuals to resubmit fingerprints. Applicants are instructed to check “Yes” to the related attestation in the facility agent application if they have submitted fingerprint as part of the facility licensing application process.

Fingerprint registration is required on the Missouri State Highway Patrol’s MACHS portal. Follow the instructions provided on the form listed below.

NOTE: During registration, applicants have the option of 1) "For Digital Fingerprinting Services (Live Scan) for In-State Applicants" or 2) "To Mail In Your Fingerprint Card -- Option 2 is ONLY for Out-Of-State Residents or Individuals Physically Unable To Be Digitally Printed. Follow the prompts to complete the registration.

Applicants may also call IDEMIA for assistance at (844)543-9712. IDEMIA can register individuals over the phone and set up an appointment at the nearest IDEMIA/IdentoGO location.

Fingerprint results will be sent to the Medical Marijuana Program for review, not the applicant.
