The Home and Community Based Services (HCBS) Web Tool is a module of Missouri Department of Social Services, MO HealthNet Division’s CyberAccess, an innovative electronic health record for participants. Conduent contracts with the Missouri Department of Social Services, MO Healthnet Division and the Department of Health and Senior Services, Division of Senior and Disability Services to provide CyberAccess.

The HCBS Web Tool provides a web portal that gives stakeholders, including HCBS providers, access to a participant’s screening, assessment(s), and prior authorization(s) for Medicaid funded HCBS.

The HCBS Web Tool seeks to improve the quality of life for participants through increased efficiency of assessments and care planning to meet a participant’s needs. CyberAccess is available to healthcare providers including HCBS providers. Medical providers can view diagnosis data, receive alerts, and review medical prior authorizations.

Provider CyberAccess Enrollment

It is critical that HCBS providers enroll in CyberAccess. Conduent manages provider enrollment in CyberAccess. New HCBS providers must complete the CyberAccess Checklist and return it to the appropriate Conduent Provider Outreach Representative.

Enrollment ensures that HCBS providers are available for selection within the appropriate provider list for HCBS.

Upon enrollment pertinent provider staff shall obtain a username and password in order to access CyberAccess and the HCBS Web Tool.

  • HCBS provider supervisors, clinical staff, field coordinators, assessors and billing staff may request access.
  • Direct care staff including in-home aides and other home care staff shall not have access.

Account Maintenance & System Settings

Conduent Provider Outreach Representatives are responsible for assigning unique usernames and passwords to the appropriately identified HCBS provider staff. Usernames and passwords shall never be shared. Users can manage their passwords on the 'My Account' tab within CyberAccess. When creating a new password, it must include a minimum of 8 characters, at least 1 uppercase, 1 lowercase, 1 number and 1 symbol.

When a user has forgotten their password, they must select 'Forgot Password' on the log in screen after entering the username. Unless the user has changed their security question, the following question will display:
'What is your favorite vacation destination?' Enter 'Hawaii.' Select 'Submit.' The user will receive a new password via the e-mail address associated with their CyberAccess account. All users are required to log in at least once every 45 days to ensure their account does not become inactive.

Providers should contact their Conduent Provider Outreach Representative for all issues related to user account maintenance including enrollment, access, password issues and adding or deleting user accounts. It is the provider’s responsibility to contact their Conduent Provider Outreach Representative when a user no longer needs access to the HCBS Web Tool, i.e. employee termination/resignation.
