Adult Brain Injury Program Providers
The Adult Brain Injury Program assists Missouri residents, ages 21 to 65, who are living with a traumatic brain injury. Rehabilitation services are provided to participants who meet financial guidelines and when it has been determined the services are necessary to facilitate a long-term goal as indicated in the Special Health Care Needs Service Plan.
Rehabilitation services include:
- Neuropsychological Evaluation and Consultation
- Adjustment Counseling
- Transitional Home and Community
- Support Training
- Pre-Vocational/Pre-Employment Training
- Supported Employment/Follow Along
- Special Instruction
Provider Enrollment and Role/Responsibility
The Provider must meet certain qualifications to be enrolled with Special Health Care Needs to provide Adult Brain Injury Program rehabilitative services. The Provider actively participates as a member of the planning team for services provided to participants, with a clear expectation of collaborating with all team members. Collaboration across programs and services is a key component of the person-centered service delivery model and is essential for effective and timely communication between all agencies and participants involved.

New Provider Enrollment, Re-Enrollment and Provider Address Change Information
Providers must review and complete the Participation Agreement and Provider Application for new enrollment, re-enrollment or when there is a change in their payment mailing address. Additionally, registration is required at MissouriBUYS, powered by MOVERS. In the event of an address change, providers must also update their information in MissouriBUYS, powered by MOVERS. Submit the completed forms via email or mail.
