SUBJECT: HCBS Policy Manual Updates

3.00 Appendix 1 Services Units and Rates

Updates include:

  • The approval of the Independent Living Waiver and the Structured Family Caregiver Waiver from Central Office to the Bureau of Federal Programs
3.25 Personal Care Assistance State Plan Consumer Directed Model

Updates include:

  • The link to the SLUMS is now a direct link to the SLUMS on the St. Louis University (SLU) website
4.00 Appendix 8 SLUMS Form and Instructions

Updates include:

  • Removed the SLUMS form and replaced it with a direct link to the SLUMS on the St. Louis University (SLU) website
  • Instructions updated to include Show-Me Home
  • Large Clock Drawing document now located under the Resources and Quick Guides sections on the Provider Reassessment and HCBS Staff Information page
4.25 Provider Reassessment Process

Updates include:

  • Added language if a participant is unable to sign or understand information on forms, the reassessor shall ensure their signature is on the form and attached to the participant's record
7.00 SLUMS Show-Me Home

Updates include:

  • Participants must meet the Nursing Facility Level of Care
  • ALFs and RCFs are now considered qualified housing that participants can transition to

Updates to Special Considerations include:

  • SMH program now has a dedicated email account that contractors must use for all payment reimbursement requests

The revised policies have been posted in the HCBS Policy Manual.
