HCBS Overview & Service Options
Home and Community-Based Services (HCBS) are programs that help older adults or people with disabilities live in their homes or communities rather than moving to a nursing home or other institution. HCBS provides services and support for people who need help with daily activities. HCBS can be provided in different settings, such as the individual's home, assisted living facilities (ALF), residential care facilities (RCF), or adult day cares. The goal is to help them live as independently as possible and stay engaged in their communities.
The Division of Senior and Disability Services (DSDS) is in charge of the HCBS programs for older adults and individuals with disabilities. The division offers a variety of services. The types of services a person can get depend on their individual needs and if they qualify. Service options may include:
- Personal Care: Help with daily tasks such as bathing, dressing, grooming, mobility, preparing meals, eating and household chores
- Advanced Personal Care: Help with more complex home care needs, for instance, transfer devices or catheter hygiene
- Meal assistance: Home-delivered meals
- Respite Care: Temporary care that provides a break to caregivers
- Home modifications: Changes to a home to improve safety and accessibility
- Transportation: Help with transportation for essential activities like grocery shopping and errands
- Adult day care: Oversight and care in a supervised setting during daytime hours
- Medical Supplies/Equipment: Items such as incontinence supplies, lift chairs and walkers
