Introducing a New Electronic Case Management System for Home and Community Based Services

To meet the demands of an ever-growing Home and Community Based Services (HCBS) population, The Division of Senior and Disability Services (DSDS) is implementing, with the assistance of Visionary Integration Professionals (VIP), a Commercial Off-the-Shelf system based on Tyler Technologies’ entellitrak©® platform. This system is scheduled to launch on May 5, 2025.

Fusion will support the entire lifecycle of HCBS, from intake and eligibility to assessment, person-centered care planning, service authorization, and other ancillary case management activities.

The entellitrak©® platform, the basis for the Fusion, provides key advantages:

  • Configurability
    • Fusion is easy to configure and execute rapid development methods since it does not require custom code.
  • Dynamic Workflow Support
    • Fusion includes a powerful built-in workflow rules engine for creating static, dynamic, and complex workflows to include backward and concurrent workflows.
  • Scalability
    • With its web-based design, multi-tier architecture, and ability to support clustered environments, Fusion is highly scalable to accommodate large volumes of users and transactions and support additional workflows.
  • Open Architecture
    • Fusion is designed on open standards with all layers of the application fully exposed for simultaneous access and data exchange with other systems. This allows MO DHSS to leverage investments in other technologies within Fusion.
  • Security
    • With its multi-factor authentication, data encryption, and role-based access controls (RBAC), Fusion securely stores large quantities of sensitive data and documents.

Combined, these technological advantages permit DSDS to provide excellent customer service to Missourians seeking to participate in, or refer others to, HCBS programs. Fusion users can quickly access participant records, assess eligibility for programs, craft person-centered plans of care, and authorize services based on participant preferences. The dynamic workflow support, scalability, and open architecture allow Fusion to grow alongside a growing aging population and ensure DSDS staff can efficiently navigate complex processes in one, consolidated digital workspace.

To prepare HCBS Providers, their staff, and other interested stakeholders for the launch of Fusion, reference materials have been posted to this site. Please review the materials to ensure understanding of Fusion and to learn more about upcoming milestones, such as training and the launch of Fusion.
