Beginning April 1, 2019 you will now be able to pay your application fees online. All Initial, Renewals, and C.H.O.Ws application fees can be paid by accessing the payment system at www.health.mo.gov, scrolling to the bottom of the home page and clicking Site A to Z under Useful Links, and click Online Payments.

An initial application will be considered as a brand new ASC with the Bureau of Ambulatory Care. Renewals will be the annual license renewal and the reminder letter will still be sent in advance of the expiration date. C.H.O.Ws (Change of Ownership) are any changes in owner or a 50% or more change in stock options.

When using the Online Payment option to pay application fees the receipt will need to accompany the application for processing. The application may now be sent by mail, fax, or email when using Online Payments. We will still accept applications and checks at the mailing address.
