Medically Fragile Adult Waiver Program (MFAW)
The Medically Fragile Adult Waiver (MFAW) Program provides service coordination and authorization for medically necessary services to MO HealthNet (fee-for-service, not managed care) recipients with serious and complex medical needs who have reached the age of 21 and are no longer eligible to receive services through the Healthy Children and Youth (HCY) Program. Participants must require medical care equivalent to the level of care received in an intermediate care facility, not be enrolled in another waiver and have been eligible for private duty nursing through the HCY Program. Authorized services may include in-home personal care, in-home nursing care, skilled nursing visits, supplies and equipment.

Program Eligibility Requirements
- Be a Missouri resident
- Be age 21 or over
- Be enrolled in MO HealthNet (fee-for-service, not managed care)
- Have a federally matched MO HealthNet eligibility code
- Require medical care equivalent to the level of care received in
- an intermediate care facility
- Not be enrolled in another waiver
- Have been eligible for private duty nursing through the Healthy Children and Youth Program
Services Available through the Program
Service Coordination is provided through Special Health Care Needs and includes:
- Evaluation and assessment of needs
- Identifying and accessing service providers
- Service plan development and implementation
- Coordination of services through resource identification and referral
- Family support
- Assisting in establishing a medical home
- Transition planning
Services authorized by Special Health Care Needs include the following and are provided by Medicaid providers:
- Private duty nursing
- Advanced personal care
- Personal care aide
- Authorized Registered Nurse visits
- Specialized medical supplies
