Reducing maternal and neonatal morbidity and mortality by encouraging growth and quality improvement efforts within facilities providing care for high-risk populations.
Risk-appropriate care, or perinatal regionalization, is a strategy to improve health outcomes for pregnant women and infants. Having a coordinated system helps to ensure pregnant women and infants at high risk of complications receive care at a birth facility that is best prepared to meet their health needs.
Designation Process for Facilities & Providers
Step 1: Gather Information
You may print a copy of the LOCATe assessment instructions and gather all necessary documentation.
Step 2: Submission
Once you have all necessary input, please click here for the LOCATe survey and complete the survey in line with the due dates listed below.

Step 3: Verification
On your LOCATe survey, you will select who will verify your maternal and/or neonatal levels of care. Once your survey has been submitted, your facility’s LOCATe assessment and level of care designations will need to be verified by DHSS, The American Academy of Pediatrics (AAP), or The Joint Commission according to the timeframes listed below.

If your facility selects DHSS, your facility will receive further communications via email. If your facility chooses to have AAP or The Joint Commission, your facility is responsible for communicating with these organizations using the following contact information.
Step 4: Additional Contacts
Register additional contacts for your facility to receive notifications from the Missouri Department of Health and Senior Services LOCATe Program.
