What is ShowMeVax?

ShowMeVax is a secure and confidential data system designed to meet the national standards for effective tracking and timely administration of immunizations in a public health setting. It is a web-based database that maintains complete, accurate, and secure immunizations records for Missouri residents. The immunization information is available to authorized users, such as health care providers, childcare facilities, and schools who work together to inhibit vaccine-preventable diseases.

ShowMeVax meets the Immunization Information System (IIS) National Standards with the following features:

• Vaccine ordering, administered dose reporting, and inventory tracking all in one system.
• Bidirectional (query and response) electronic data exchange with electronic health record systems (EHRs).
• Immunization Coverage Rate Reports
• Up to date programming, called the “Recommender”, which forecasts vaccinations that are invalid, due, and overdue to help avoid under and over-vaccinations.
• Reminder/Recall Reports
• Official immunization certificates for schools, childcare, camps, and even college.
• Electronic querying for patients’ previous immunization records.

Tips for Locating an Immunization Record

Finding old immunization records can be difficult.

• Check with your health care provider.
• Contact previous health care providers. Don't forget visits to your local public health agency or other clinic.
• Look through old papers--sometimes immunization records are tucked away in a baby book, included on school or camp medical history forms or in safes.
• Check with any schools or child care programs your child has attended to see if they have retained a record of immunizations required for school entrance.
• Ask your high school, post-secondary school, college health service, or previous employers (including the military) for dates of any immunizations, if applicable.
• Millions of immunization records are stored in ShowMeVax, a web-based electronic database. Those wishing to access their personal and/or family immunization records can use Docket® to access their ShowMeVax records.

About Immunization Records and Docket®

Missouri residents can use Docket® to access personal and/or family immunization records on file with the Missouri Immunization Registry (ShowMeVax). To access your ShowMeVax record(s), you must have a valid phone number and/or email address on file with your ShowMeVax record. In addition, the name, date of birth, and legal sex you enter in Docket® must match your ShowMeVax record exactly.

Using Docket®

Docket® is available to download at no cost on the Apple App Store or Google Play Store. You may also access Docket® on the web at Docket. Use of Docket® is 100% optional.

1. Set up your Docket® account
   Sign up or log in to Docket® using your email address, Apple ID, or Gmail account. 
2. Search ShowMeVax immunization record(s)
   Your name, date of birth, and legal sex entered in Docket® must match your ShowMeVax record exactly. Phone number (via text message or robocall) or email address verification is required to access a digital copy of your personal and/or family immunization record.
3. Manage your records
   1. Access back-to-school forms
      Select the PDF icon to access a PDF copy of your official Missouri immunization record. You can export it from Docket® using standard share functionality (e.g., email, text, print, etc.).
   2. Get immunization reminders
      Enable notifications to receive reminders for upcoming and overdue recommended immunizations based on public health guidelines. Click the calendar icons to add all-day reminders to your Apple, Google, and/or Outlook calendar apps.
   3. Add family members
      Tap the plus (+) icon on the Immunization Search History screen to add family records to a shared Docket® account.

It is important to save any information you discover in writing, including the vaccine name (e.g., "MMR"), date given, and provider or clinic name. Utilize the immunization record as a lifelong record of an individual’s immunization record.

• Remember to bring the immunization record card to all medical appointments.
• Be sure all immunization providers give you a written record of the vaccine(s) you received.
• Keep your immunization record in a safe place. You may need it throughout your life.
