How do I submit the required immunization data for my school district?
If you would like to report online you can complete the online report on the website at https://health.mo.gov/living/wellness/immunizations/schoolrequirements.php, under School Reporting Information. The first link “Online School Reporting” is the link to report online. Remember to save often as you are entering your data. You can save by going to the bottom of the form and clicking on “Save”. Once you have entered your entire report you will need to scroll to the bottom and click on “Submit”.
If you would like to submit a paper copy you can print the report from the same website under School Reporting Information.
