To Apply for a CNA Instructor or CMT Instructor/Examiner Train the Trainer.
If you are wanting to become a CNA Instructor and a CMT Instructor/ Examiner, complete a separate request for each.

The applicant must provide documentation showing these requirements are fulfilled by submitting the following to cnaregistry@health.mo.gov:

  • Resume
  • Copy of Nursing License
  • Copy of Social Security Card (write out number if not legible after copied)
  • Address and Email Address
  • Phone Number (if available)

DHSS will evaluate and verify the applicant's credentials and will notify the individual in writing within seven (7) to ten (10) working days regarding the request.

An applicant must attend the state approved instructor train-the-trainer course. When the individual has successfully completed the course their name will be added to the department's CNA Instructor Registry. You are responsible for updating your personal information contained on the registry. You may change your personal information contained on the registry. You may change your personal information by completing the Registry Update form.

CNA Instructor

Individuals wanting to be certified as a CNA Instructor must be approved by the DHSS to attend the train-the-trainer course. This training course is conducted by a Long-Term Care Association.

The applicant must meet the following qualifications to become an instructor:

  • A Registered Nurse (RN) licensed in Missouri or in possession of a temporary permit from the Missouri State Board of Nursing. The licensee shall not be subject to current disciplinary action; and
  • A minimum of two (2) years of nursing experience as a RN and at least one (1) year experience in provision of LTC facility services within the last five (5) years (Please be specific in providing your experience).

CNA Examiner

Contact Headmaster D & S to become a Test Observer at 1-800-393-8664. Headmaster D & S is the third party company that provides the testing for Missouri CNA students.

CNA Clinical Supervisor

Individuals wanting to be certified as a CNA Clinical Supervisor must attend the train-the-trainer course. This training course is conducted by a Long-Term Care Association.

The applicant must meet the following qualifications to become a clinical supervisor:

  • The individual must be a Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed in Missouri or in possession of a temporary permit from the Missouri State Board of Nursing. The licensee shall not be subject to current disciplinary action; and
  • At a minimum, have one year of Long Term Care (LTC) experience or currently employed by a LTC facility; and
  • The nurse must attend the state approved train-the-trainer course.

When the individual has successfully met the clinical supervisor qualifications, their name will be added to the Department's CNA Clinical Supervisor Registry. You are responsible for updating your personal information contained on the registry. You may change your personal information by completing the Registry Update form.

CMT Instructor/Examiner

Hold a RN in Missouri or shall have a temporary permit from the Missouri State Board of Nursing. The instructor/examiner shall not be the subject of current disciplinary action, such as probation, suspension, or revocation of license;

  • Shall complete an instructor/examiner program workshop and be listed as a qualified CMT instructor/examiner on the department’s Instructor/Examiner Registry;
  • Shall sign an agreement with the department to protect and keep secure the final examination and the PIN used to electronically access the Instructor Guide/Test Bank;
  • May be an employee of the ICF/SNF in which training is conducted, but the ICF/SNF must have a cooperative agreement with an educational training agency;
  • Shall teach the course or facilitate the challenge examination only as permitted by the educational training agency; and
  • May be assisted by pharmacists as guest instructors in the areas of medication systems, regulations governing medications, medication actions, adverse reactions, medication interactions, and medication errors.

Level I Medication Aide (LIMA) Instructor

Individuals wanting to be certified as a LIMA Instructor must attend the train-the-trainer course. This training course is conducted by a Long-Term Care Association.

The applicant’s license shall not be subject to current disciplinary action, and the individual shall not be on the Employee Disqualification List. The applicant must also meet the following qualifications to become a LIMA Instructor:

  • A Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed in Missouri or in possession of a temporary permit from the Missouri State Board of Nursing, or shall hold a multi-state or single state registered nurse license from a jurisdiction that is party to the Nurse Licensure Compact; and
  • A minimum of one (1) year experience working as a nurse.

The applicant must provide documentation showing these requirements and be approved by the Association you will be attending the workshop through.

When the individual has successfully completed the train-the-trainer course their name will be added to the Department's LIMA Instructor Registry. You are responsible for updating your personal information contained on the registry. You may change your personal information by completing the Registry Update form.
