Persons are encouraged to contact the CON team early in the project planning process.
All forms are interactive PDF forms, which means they can be typed into, check boxes may be checked and numerical fields are self-calculating. Once completed, forms need to be printed, signed (where applicable), included with other applications or forms and sent to the Certificate of Need program office via email or mail. If problems are encountered, please contact a member of the CON team.
CON is currently utilizing 2030 projected population data. To obtain population for your project, fill out the Missouri Population & Data Form located within the Application & Other Forms link below. 2035 population data will be available closer to 2029-2030.
