Actions to be taken to obtain DOT-SP 10656 Exemption for radioactive material
In the event that radioactive material is detected in a load of scrap metal, a DOT exemption can be obtained by visiting the web link https://health.mo.gov/emergencies/ert/haz-info.php#rad and downloading a copy of the exemption form and filling out section 1, 2, and 3 as completely as possible. The form can then be attached and sent via email to moradhealth@health.mo.gov. In addition, please call the following number to inform the duty officer that the email has been sent 1-800-392-0272.
Upon receipt of the form, it will be reviewed and if there are any questions you will be contacted by State Health Department Personnel. In most cases, the form will be completed and a SP10656 approval number will be assigned. The form will be returned to you for your records. A copy of the form must be given to the carrier/driver before they return to the point of origin.
The Missouri Department of Health and Senior Services has been identified by the Conference of Radiation Control Program Directors as the authority for the issuance of the SP-10656 approval number.
