Memorandum for Home and Community-Based Services Staff and Stakeholders

SUBJECT: REDCap Outage

This memorandum is to advise Home and Community Based Services (HCBS) staff and stakeholders that REDCap will be unavailable on Wednesday, May 6, 2026, from 4 pm to 7 pm for an update. We apologize for any inconvenience this may cause.

This outage will impact the following REDCap Portals:

  • All Review Tools (PRR, DSDS, PCCP, and Waivers)
  • Fusion Access Request Form
  • Ombudsman Discharge Notice Portal
  • SHCN Monthly Contact Survey
  • Provider Reassessor Exams

This outage will not impact the ability to submit referrals for HCBS or PCCP requests.
