CLIA Waiver

If your testing plan includes administering tests in a congregate care setting, you must do so under a CLIA waiver. You may use your own CLIA waiver or the state CLIA waiver (if you don’t have your own). Using the state CLIA waiver requires you provide verification of testing locations, validation of training of all test administrators, and submission of a testing plan inclusive of the infectious waste disposal plan. You will provide this information during the application process. Congregate care facilities include but are not limited to: long-term care facilities, mental health facilities, hospitals and veteran’s homes, child care facilities, schools, universities, and summer camps.

Standing Orders

Effective May 12, 2023, each partner/provider participating in the state’s antigen testing program must provide their own physician standing order for all tests administered in a congregate care setting. This includes but is not limited to, long-term care facilities, mental health facilities, hospitals and veteran’s homes, childcare facilities, schools, universities, and summer camps.

The DHSS Rapid Antigen Test Kit Application and Resource web page includes the initial application and a subject index with important links to product support/information, link to the order form, templates for testing plans, etc. A training tutorial to complete the application is available here. CLIA Waiver

Note: For organizations using their own CLIA, be sure to add ACON Flowflex and LifeSign Status COVID-19/Flu A&B to your CLIA waiver by contacting the state CLIA program.

LifeSign Status COVID-19/Flu A&B Antigen Test

The COVID-19/Flu A&B combo test kits are now available through the state’s antigen testing program. These test kits must be administered under a CLIA waiver and in a congregate care setting by testing professionals. They are not distributed for at-home use. DHSS is providing standing orders for the combo test kit ONLY. The combo test kit has also been added to the state CLIA waiver. If you are using your own CLIA waiver you will need to add the combo test kit (if you are receiving these from the state), by contacting the state CLIA waiver program at antigentesting@health.mo.gov.
