According to the Missouri State Code of Regulations, licensed ambulatory surgical centers (ASCs) are required by statue (Section 192.667, RSMo.) to report Healthcare associated infection data to the Missouri Department of Health and Senior Services as written in (19 CSR 10-33.050 -- see page 35). This regulation requires Missouri licensed ambulatory surgery centers (ASCs) to submit information to the Missouri Department of Health and Senior Services (MDHSS) on an annual basis, for the purpose of determining their facility’s SSI related reporting/exemption status. To facilitate this requirement, the department has developed a web-based application for the annual registration process.

Annual registration takes place each year from January 1 through March 31 and must be completed by all licensed ASCs even if breast and hernia procedures are not performed in your surgery center. Registration must be completed no later than March 31st of each year.

To access the MHIRS annual registration form, the healthcare facility must log on to the registration site https://healthapps.dhss.mo.gov/mhirsregistration/. The login screen will prompt users to enter their email address and enter the User ID and PIN assigned to the ASC. The user ID and PIN will be emailed to each facility administrator at the beginning of the year. Users should not attempt to register multiple facilities using the same MHIRS ID and PIN. Please submit one registration for each licensed ASC; the MHIRS ID and PINs are different for each ASC.

In preparation for registration, users will be asked to assess the number of breast and hernia surgeries performed in their ASC during the previous year. This preliminary information will be used to assess whether or not the facility will have data to report for the current year. Users will also be asked to provide contact information (Name, email, and telephone) for at least two unique contacts.

  • Facility Administrator: This is the acting or current ASC administrator. An administrator can additionally serve as a Primary or Secondary contact.
  • Primary Contact: This should be the person you want the DHSS to contact regarding infection data reporting issues and requirements.
  • Secondary Contact: This is the person you want the DHSS to contact in the event the Primary Contact is unavailable.

When entering registration data, the email address provided by the registrant will serve as a signature of the person completing the registration. Once you have completed the registration process and selected the “Finalize” button in the last registration screen, you and the registration contacts will receive an email containing an annual registration confirmation and a preliminary exemption/reporting status for the current calendar year. The term “preliminary is used because the reporting status can change depending on the number of surgeries performed in the current year. If 20 or more breast or 20 or more hernia procedures are performed during the current year, than you must report data for that particular surgery type.
