The data required to be provided by each health maintenance organization is collected under three tables:

  • Table A (Member Satisfaction Survey)
  • Table B (Quality of Care Indicators)
  • Table D (Services Offered/Access to Care for Members)

Tables listed below contain information for all plans who provided data in compliance with state law. Reports for Table A are broken out for the separate plans while Table B and Table D contain data for all plans on one spreadsheet. Table B and Table D can be filtered/organized on specific questions/measures or filtered on which plans you wish to view.
