Patient - Rejected Application Information

Ensuring Patient Application is Complete

If the department reviews an application and determines it is incomplete, per 19 CSR 100-1.040 (6)(B)5.B it will be returned to the applicant for corrections.
A complete application is an application that includes all information required by 19 CSR 100-1.040. The department will notify an applicant once if an application is incomplete and will specify in that notification what information is missing. If an applicant fails to provide a complete application within fourteen (14) calendar days of being notified that an application is incomplete, the card for which the applicant is applying will be denied.
The Department cannot accept documents received through email or outside of the online registry portal. All documents must be submitted with an application, through the online registry portal. When making corrections to an application, be sure to click the “SAVE and NEXT” button to save changes before resubmitting the application to the Department.
Reminder: After uploading files or editing data, be sure you click the “Save” button to commit your changes before submitting or resubmitting your application to the department.

Common Reasons Renewal Applications are Returned for Corrections:

Currently, the Department is seeing trends in renewals being returned for corrections and would like to remind applicants of requirements that are often missed. Renewal applications will be returned if the following requirements are not met:
- A color digital photo of the applicant, taken within the previous 3 months. This must not be a photo of the applicant’s driver’s license or passport.
- Government Issued Photo ID
- Electronic Physician Certification Form signed less than 30 days before the application’s submission date.
Cultivation details must include location and security of the cultivation space.
If a cultivation space is being shared, the name and Patient ID card number (PAT#) of the one additional patient who will be sharing the space must be listed. If the individual the applicant intends to share with is applying for cultivation or for a caregiver license, applicants need to type “applying for cultivation.” In the PAT # space.

Patient - Denied Application Information

Common Reasons for Application Denial

Pursuant to 19 CSR 100-1.040 (6)(B)5, Qualifying patient, primary caregiver, and patient cultivation identification cards may be denied.

If an applicant provides false or misleading information in an application, the identification card for which the applicant is applying will be denied.
If an applicant fails to provide a complete application within ten (10) days of being notified that an application is incomplete, the identification card for which the applicant is applying will be denied.

(I) An applicant will be considered notified on the date the department sends a written explanation of how the application is incomplete to an e-mail address provided by the applicant.

Patient Appeals and Administrative Penalties for Card Holders

Pursuant to 19 CSR 100-1.020 (4), denials of identification cards may be appealed to the Administrative Hearing Commission (AHC) within thirty (30) days after the date of this notice. Appeals may be sent by regular or certified mail to: The Administrative Hearing Commission, P.O. Box 1557, Jefferson City, MO 65102-1557; or sent via fax to 573-751-5018. Pursuant to 19 CSR 100-1.040(6)(C) – There are administrative penalties for card holders. Qualifying patient, primary caregiver, and cultivation identification cards may be sanctioned. Please visit 19 CSR 100-1, section 040 (6), subsection (C) of the rules for more details of administrative penalties.
