Memorandum for Home and Community-Based Services Staff and Stakeholders

SUBJECT: Bureau of Policy and Quality Enhancements
This is to inform Home and Community Based Services (HCBS) provider reassessors that all required forms and documents (e.g., HIPAA, guardianship, and power of attorney (POA) documents, etc.) have been migrated to Fusion.
During a reassessment, if a provider reassessor identifies that any required form(s) are missing from the participant’s case record, they shall obtain the form(s) with the appropriate signature(s) from the participant and attach them to their case record in Fusion.
Thank you for your attention to this matter and for ensuring complete and accurate participant records.
