SUBJECT: Brain Injury Waiver
This memorandum is to notify Home and Community Based Services (HCBS) staff and stakeholders of the newly funded Medicaid waiver: Brain Injury Waiver (BIW).
Prospective providers must enroll as a Type 85 provider with Missouri Medicaid Audit and Compliance (MMAC), complete Brain Injury Waiver Participation Agreement and Exhibit 1 – Business Entity Certification, Enrollment Documentation, and Affidavit of Work Authorization

This revised policy has been posted in the HCBS Policy Manual.
