MEMORANDUM FOR HOME AND COMMUNITY BASED SERVICES STAFF AND STAKEHOLDERS

FROM:              Melainie Highland, Director, Division of Senior and Disability Services

SUBJECT:        State of the Workforce - Aging and Disabilities 2025 Survey Instructions

This memorandum serves as a reminder to all Home and Community-Based Services (HCBS) providers of the upcoming State of the Workforce—Aging and Disabilities (SoTW‑AD) Survey, which begins on October 1, 2026. Providers should thoroughly review all instructions and communications from both the Division of Senior and Disability Services (DSDS) and the survey administrators before beginning the survey.

The SoTW‑AD Survey focuses on HCBS providers that delivered Agency Model Personal Care Services (In‑Home or Residential Care Facility/Assisted Living Facility) and Adult Day Care Services for the full 2025 calendar year through DSDS. Consumer‑Directed Services (CDS) providers are not included in this survey. A separate CDS-focused survey will be released in January 2027. Providers who are dually enrolled as a CDS provider or as another provider type, such as Home Health or Department of Mental Health Services, must isolate Agency Model Personal Care or Adult Day Care Services data when reporting.

ADvancing States and HSRI will administer the survey. The survey will be distributed via email and completed online using the Verity Analytics link included in your email invitation. Invitations will be sent to the business email address on file with Missouri Medicaid Audit and Compliance (MMAC). Invitations will go out the week of October 1, 2026, and the survey will remain open through November 30, 2026. Providers who do not receive an invitation by October 2, 2026, and believe they should have, should notify DSDS at DSDS.Surveys@health.mo.gov after checking their spam/junk folders.

Because the survey is comprehensive, providers are strongly encouraged to open and begin reviewing it immediately to ensure all required information is available. Accurate reporting is essential, as the data collected will inform future HCBS fiscal planning and programmatic changes. If providers have questions or need clarification about how to answer any survey questions, they should reach out to DSDS at DSDS.Surveys@health.mo.gov.

This will be Missouri’s fifth year participating in the SoTW‑AD Survey. Last year, approximately 25% of providers responded. While the information received was valuable, DSDS hopes to see even greater participation this year. Your input is critical in shaping future HCBS initiatives, policies, and improvements.

This memo is issued in addition to INFO 07-26-01. Providers should review those communications for further details. All questions regarding this memorandum or the SoTW‑AD Survey should be directed to DSDS at DSDS.Surveys@health.mo.gov.
