For Missouri Department of Health and Senior Services and Contract County permits only:
Prior to completing the Construction Permit Application, you must obtain an application number. Once you complete the OWTS Application Number Request Form, you will receive an email with your application number and a link to the new electronic Construction Permit Application. If hardship prevents you from completing the electronic Construction Permit Application, we will provide instructions for mailing in the application and supporting documents within the email.
With the new electronic Construction Permit Application, you will attach your soil evaluation report, engineer designs, and other documents relating to the OWTS construction to the appropriate permitting authority.
Due to the temporary suspension of permit fees, you will not need to provide any permit fee receipt.
