The Medically Fragile Adult Waiver Program provides service coordination and authorization for medically necessary services to MO HealthNet fee-for-service recipients with serious and complex medical needs who have reached the age of 21 and are no longer eligible to receive services through the Healthy Children and Youth Program. Individuals must require medical care equivalent to the level of care received in an intermediate care facility, not be enrolled in another waiver and have been eligible for private duty nursing through the Healthy Children and Youth Program. Authorized services may include in-home personal care, in-home nursing care, and specialized medical supplies.

Medically Fragile Adult Waiver Eligibility
Eligibility
The participant must:
- Be a Missouri resident
- Be age 21 and over
- Be enrolled in MO HealthNet (fee-for-service)
- Have a federally-matched MO HealthNet eligibility code
- Require medical care equivalent to the level of care received in an intermediate care facility
- Not be receiving services from another state waiver program
- Have participated or been eligible for private duty nursing through the HCY Program

Medically Fragile Adult Waiver Service Coordination
Service Coordination consists of:
- Assisting recipients/families in identifying and accessing medically necessary services
- Establishing a medical home
- Connecting families with appropriate community resources
- Meeting with the recipient and family on an ongoing basis to plan, promote, assist and assure the implementation of services
Medically Fragile Adult Waiver Prior Authorization
The SHCN Service Coordinator authorizes the following services:
- Private duty nursing
- Advanced personal care
- Personal care aide
- Authorized Registered Nurse visits
- Specialized medical supplies
