MCCY/MCAW Referral Process

MCCY/MCAW Referral Process

Referrals can be taken by any SHCN staff to ensure that someone is not directed to several different places, delaying the process. MO HealthNet eligibility will be determined by the Facilitator. A Service Coordinator (SC) must review all appropriate MCCY and MCAW referrals with straight Fee-For-Service (FFS) Medicaid to determine medical eligibility for enrollment into paid services. The MCCY/MCAW Program Manager (PM) must approve all MCAW referrals prior to paid service enrollment. Please reference the MOHSAIC User Guide, eMOMED Guide and HealthTrack at a Glance resources for system assistance and the MCCY & MCAW Guidebooks for additional information on referral, initial enrollment, program eligibility and records management.

1B-Regional Office

  • Complete the MCCY & MCAW Referral Form to ensure that all applicable information is obtained from the referral source.
  • If the referral is received by Regional Office staff other than the Facilitator, forward the MCCY & MCAW Referral Form to the corresponding Facilitator for processing.
