ALL Missouri Department of Health and Senior Services (DHSS) SUBRECIPIENTS are required to submit one Subrecipient Information Form (SIF), regardless of the number of contracts or agreements they have with DHSS or the number of business locations. Completion of the SIF helps ensure compliance with 2 CFR 200, the Federal Funding Accountability and Transparency Act of 2006 (FFATA), and the Health Insurance Portability and Accountability Act (HIPAA) of 1996.

New Subrecipients
If you are new to doing business with DHSS or you do not have a SIF on file, click the “Subrecipient Information Form (SIF)” button above and select “New Subrecipient” to complete the SIF.
To check whether your organization has previously submitted a SIF, click the “Public Subrecipient Report” button below and search for your organization. If your organization appears in the report, it is considered a returning subrecipient. (See below for information on how to update your SIF or request a copy.)

- Executive Director, Board President
- Primary Financial Executive
- Your organization's federal taxpayer ID number (FEIN/EIN)
- Your organization's Unique Entity Identifier (UEI) from SAM.gov. (See below for more information on UEI & SAM.gov.)
- Your organization's Zip+4. Find your 9 digit zip code
- Your organization’s Congressional District. Find your district
- Contact information for your organization’s Financial Officer, Privacy Officer and Certifying Official
Returning Subrecipients
To update your information or request a copy of your SIF, follow these steps:
- Click the “Subrecipient Information Form (SIF)” button above
- Choose “Returning Subrecipient.”
- Select your desired action.
- Enter your UEI, FEIN, Financial Contact Name, and Financial Contact Email.
- Select “Send me a copy of my responses” at the bottom of the page, if desired.
- Click “Submit.”
Unique Entity Identifier (UEI)
A UEI is required to complete the SIF because, in 2022, the federal government transitioned from using the Data Universal Numbering System (DUNS) to the Unique Entity Identifier (UEI), new non-proprietary identifier issued at SAM.gov.
If your organization is a registered entity with SAM.gov:
