The Division of Senior and Disability Services (DSDS) is responsible for the administration of Home and Community Based Services (HCBS) including the following: intake of referrals, conduct assessments of nursing facility level of care (LOC) and authorization of services through person centered care planning.

Referrals

Home and Community Based Services Providers can submit new referrals using the Online Home & Community Based Services Referral Form or by sending an HCBS-1 Form to HCBSCallCenterReferrals@health.mo.gov.

Participants, family, friends, or informal supports can submit new referrals by phone at 866-835-3505, using the Online Home & Community Based Services Referral Form, or by sending an HCBS-1 Form to HCBSCallCenterReferrals@health.mo.gov.

The phone line is reserved for participants and their supports only. This is an effort to ensure accessibility to participants, whom may not have other means to contact DSDS.

Policy and Procedure

The DSDS will contact each person that is referred to make sure the person is eligible for the services. DSDS’s operations are guided by policies and procedures outlined in the Home and Community Based Services manual.
