The Home and Community Based Services (HCBS) Web Tool is a module of Missouri Department of Social Services, MO HealthNet Division’s CyberAccesssm, an innovative electronic health record for participants. The HCBS Web Tool provides a web portal that gives stakeholders including HCBS providers access to a participant’s screening, assessment(s), and prior authorization(s) for Medicaid funded HCBS. As HCBS participants are transitioned from the other Division of Senior and Disability Services systems (LTACS/CDS Database) their information will be available in the HCBS Web Tool.
The HCBS Web Tool seeks to improve the quality of life for participants through increased efficiency of assessments and care planning to meet a participant’s needs. CyberAccesssm is available to healthcare providers including HCBS providers. Medical providers can view diagnosis data, receive alerts, and review medical prior authorizations.
Xerox Heritage, LLC contracts with the Missouri Department of Social Services, MO HealthNet Division and the Department of Health and Senior Services, Division of Senior and Disability Services to provide CyberAccesssm.
