Carrying Out the Plan

You’ve done your Research and completed your Action plan. Now it’s time to Carry out, or implement, the plan to help the people with diabetes or at risk for diabetes in your community.

Coordinate Activities

You included timelines in your objectives and action steps, so now schedule and hold those events. Gather together the people and resources needed to accomplish what you set out to do. Recruit new volunteers to help out at certain times or to become part of the team.

Hold Meetings and Begin Evaluating

Hold regular meetings with the members of your team and members of any committees that were created. At each meeting, take a look at where you are on your plan and timelines, and how much further you have to go. Record minutes of the meeting to show progress and be accountable.

Your evaluation will also help you determine if you need to make any changes to your plans as you go. Evaluation of a project in progress is called formative evaluation.

A quick note: You may be interested in mapping out the steps in your project using a logic model. The W.K. Kellogg Foundation defines a logic model as “a systematic and visual way to present and share your understanding of the relationships among the resources you have to operate your program, the activities you plan, and the changes or results you hope to achieve.” To learn more about the logic model process, visit the W.K.
