SUBJECT 1: From the WOM - Self-Declaration Statement for Pending Proofs ER# ER# 3.01800, ER# 3.02000, and ER# 3.03850
Question: If an applicant is missing a proof at the certification appointment, does a signed statement have to be scanned into MOWINS?

Answer: No. If a participant is missing one of the three proofs (income, identity or residency), they would self-declare that the missing proof meets eligibility requirements. The WIC-10 (11-15 and 03-16 versions) contains the self-declaration statement below:

“If all documentation is not available at certification, I self-declare that the missing income, identity or residency meets eligibility requirements. I agree to furnish it within 30 days to remain on the program and receive benefits. This certification information is being made in connection with the receipt of Federal assistance. Program officials may verify this information.”

The applicant must be notified that this self-declaration statement is part of what they are signing in the Rights and Responsibilities.

JOB POSTING: WIC Health Education Supervisor - St. Louis County Department of Public Health. The St. Louis County Department of Public Health has an opening for a Health Education Supervisor to work in the WIC Program. This is a full-time, grant-funded Registered Dietitian position working out of a North County location. Primary responsibilities include supervising the operation of a WIC office comprised of Office Service Representatives; Nutritionists; and a Breastfeeding Peer Counselor, as well as, completing the job functions of a WIC nutritionist. The Health Education Supervisor requires 2 years of Registered Dietitian experience preferably in WIC along with supervisory experience. Applications will be accepted through February 23, 2016.
