  1. WIC Exit Counseling Handout (#712) - New Look: The WIC Exit Counseling handout has been revised. All local WIC providers should have received a supply of this handout the week of April 9, 2012. Please feel free to order more using the WIC 7 form as needed. The handout in 16 languages is available on the WIC website.

    The Spanish version will be available at the DHSS warehouse by May 15, 2012. If you have any questions, please feel free to contact Takako Tagami at Takako.Tagami@health.mo.gov.

  2. Zero Income Affidavit Form: The Zero Income Affidavit Form has been translated into 16 languages and is available through the WIC website. Please make copies as needed. If you have any questions, please feel free to contact Takako Tagami at Takako.Tagami@health.mo.gov.

  3. New WIC Foods Home Page: The new WIC Foods page is available on the WIC website. This site was established in order to improve accessibility to information related to the Missouri WIC Approved Food Lists. Please visit the WIC website below for the following resources.

  4. May WIC Invoices: WIC Invoices for May 2012, and prior months, submitted for payment in the month of June must be submitted through the Online WIC Invoicing System by close of business on Friday, June 8, 2012. Invoices submitted after that date will be held and paid in July, 2012. This change in schedule complies with the close of State Fiscal Year 2012 payment processing deadlines.
