SUBJECT 1: Zero Income Affidavit

  1. Zero Income Affidavit:  We have created a Zero Income Affidavit to be used since the Multi-Purpose Affidavit that is in MOWINS is no longer available.  The Zero Income Affidavit is to be used when participants say they have no income.  The participant must be asked for information as to how they obtain food, shelter, clothing, medical care, etc. The participant must complete and sign the affidavit.  The signed affidavit must be scanned and saved in the participant folder in MOWINS.  We will change the Income Assessment policy, ER # 3.02000, to refer to this affidavit instead of the MOWINS Multi-Purpose Affidavit.  The Zero Income Affidavit is attached and on the WIC web site.

  2. Children’s Health Webinar Series:  The state WIC office was just notified of a series of webinars addressing children’s health.  The title of the series is Improving Children’s Health through Federal Collaboration.  The webinars are free and open to all interested parties but registration is required.  Attached is the time and dates of the webinars.  Please note the listed sessions are in Mountain Time.  The webinars were organized by HRSA and EPA.   3. Nutrition and Feeding Interventions for Autism, Asperger’s and ADHD:  Elizabeth Strickland, MS, RD, LD, author of Eating for Autism, is coming to Springfield, Missouri on December 9, 2011 to present a 1-day seminar, “Nutrition and Feeding Interventions for Autism and ADHD.  Registration information is attached and available through Cross Country Education, (800) 397-0180, or on the web at  Information about the speaker, Elizabeth Strickland, is available at  (ONE ATTACHMENT)

  4. FFY 2011 End-of-Year Invoicing due December 9, 2011:  The deadline for submission of FFY 2011 end-of-year invoices is Friday, December 9, 2011.  Expenses must have been incurred prior to October 1, 2011 to qualify for the FFY 2011 funding year.  The grant year will close the end of December and payments beyond this date are not possible.
