  1. Required Attachments for the FFY 2011 LAPThere has been some confusion about required agency-specific documents and policy attachments to the LAP.
    Required Attachments for the FFY2011 LAP are:
    1. Contracts (If Applicable)
    2. Indirect Cost Rate Letter (If Applicable)
    3. Outreach Plan
    4. Interpretive Services Policy
    5. Complaint & Grievance Policy
    6. Breast Pump Policy and Agreement (Select Other and designate Breast Pump Policy in the name field)

    To attach a document, select an Attachment Type, type the Document Name, and use the Browse button to find the Document Path (the location on your computer where you saved the document). Only staff with agency-level access (not data entry-level access) will be able to see the Attachment page in the LAP.

  2. Microcuvettes & Analyzers- Response Requested:  It is time to order microcuvettes by completing the survey.  Please follow the instructions below to assure your agency receives your supply of microcuvettes and analyzer.
      1. Microcuvette Survey:  Please complete and return the attached survey via e-mail to WICOperations@health.mo.gov by September 13, 2010.  This will assure no interruption in the supply of microcuvettes.
      2. Microcuvette Order:  The information from the survey will be used to identify the current supply on hand at each agency and to estimate the needs of each agency for the next six months.  The microcuvette order will be shipped directly to your agency.
      3. Analyzers:  Analyzers are also available to agencies by completing the request and justification.  Tell us why you need the analyzer: replace current analyzer; expand services at another room/satellite site; more efficient work flow; other: explain.

    Local WIC Provider Confirmation:  When you receive the order please do the following:

    1.  Verify the number of microcuvettes received with the number listed on the packing slip.
    2. If the number of microcuvettes in the package is the same as the packing slip, sign and date the packing slip.
    3. Fax the packing slip to Leslie Worrell at the State WIC Office within one (1) week of receiving the shipment.  The fax number is (573) 526-1470.  Confirmation on the packing slip is required in order for the State WIC Office to pay for the microcuvettes.  Confirmation by e-mail will not be accepted.
    4. If the number of microcuvettes received is different from the packing slip, please call Leslie Worrell at 800-392-8209 or (573) 526-4622.  Thank you for your cooperation. 

  3. Mandatory Revision 10 Risk Factor Training Webinar:  The Revision 10 Risk Factor training (pre-recorded Webinar) has been posted.  The training materials for this webinar are located here.

    Post training conference calls (optional) scheduled in September by districts will allow local agencies to ask questions regarding the Revision 10 implementation.  If you cannot participate in the call scheduled for your district, you can choose to participate in another call. Following is the schedule:
    September 9, 2010:  9 – 11 AM (Eastern District)
    September 9, 2010:  1 – 3 PM (Central District)
    September 15, 2010:  9 – 11 AM (Southeast District)
    September 15, 2010:  1 – 3 PM (Southwest District)
    September 16, 2010:  9 – 11 AM (Northwest District)
    September 16, 2010:  1 – 3 PM (Northeast District)

WIC Resource Item Shipping Schedule:  The table below is a shipping schedule for WIC resource items.  Please note that the DHSS warehouse will begin shipping them to local WIC providers as indicated in the table.  Please share the information with your staff.  If you have any questions, please feel free to contact Takako.Tagami@health.mo.gov.

The Independence City Health Department, in collaboration with DHSS, is initiating a pilot program for a Nutritionist to provide resources and education, particularly for WIC clients, including breastfeeding support for a select group of local public health agencies.
