Maintenance Reports

Maintenance reports are listed by the serial number of the instrument, followed by the date the maintenance check was performed.  For example, a maintenance check for Intoxilyzer 5000, serial no. 66005170, performed April 26, 2017, would be filed as “66005170 042617” in the 2017 link below.

Note: Mistakes in file names have been known to occur. If a maintenance report is not found for the instrument and/or date in question, contact the breath alcohol office at (573) 840-9140 or by email (BreathAlcohol@health.mo.gov).
