Call for Presentations – Submission deadline January 25, 2013

The Missouri Coalition Celebrating Care Continuum Change (MC5) invites you to submit a proposal to present at the 2013 Culture Change Conference.  This conference will be held at the Hilton Garden Inn in Columbia, Missouri, April 30, 2013 (pre-conference intensives) and May 1 – 2, 2013 (conference).  The conference will focus on ways to implement Culture Change and Person Centered Care throughout Missouri.  Submissions should reflect this year’s theme:  “Bloom Where You Are Planted.”  Submit your proposal to Zoe Dearing at zdearing@alz.org or fax to 314-432-3824.  If you have any questions, you may call Zoe at 314-801-0444.  For more details, please visit the MC5 website:  2013 Culture Change Conference.
