The Fourth Quarter 2012 Certificate of Need (CON) Quarterly Survey must be completed and returned by January 15, 2013 per CSR 30-82.010(8).  Please note that CON law, Sections 197.300 to 197.366 RSMo, requires that you report your occupancy data accurately and in a timely fashion.  If you have any questions regarding the survey, please contact the Section for Long-Term Care Regulation at 573-526-8514.
